Practitioners who provide services at or through RFWC may access your personal health information as part of your care and do so as agents of RFWC, in accordance with PHIPA and applicable professional standards.
Client records created, used, or maintained within RFWC systems or premises remain under the custody and control of RFWC as the Health Information Custodian.
Where a client requests access to or transfer of their records, RFWC will manage the request in accordance with PHIPA and applicable professional requirements.
